Screen for Fall Risk

Knowing your personal risk of falling is important so that you know the best steps to take to reduce that risk. Take the Falls Free CheckUp to learn your risk. Be sure to share this information with your healthcare provider, especially if you are at high risk of falling. They can recommend effective steps to take such as going to a physical therapist.

A quick way to find out if you're at risk for falling is to take the National Council on Aging's Falls Free CheckUp. This short quiz takes just a few minutes to complete. It will tell you if you are at risk and let you save or print a report that you can share with your healthcare provider.

You can do the screening anytime if you don't have time to take it right now. You can also use the following three yes-or-no questions to get a quick idea of whether you're at risk of a fall in the next year:

  1. Have you fallen in the past year?
  2. Do you feel unsteady when standing or walking?
  3. Do you worry about falling?

Answering yes to any of these means that you're probably at risk of falling in the next year.

Steps You Can Take
  • Take the Falls Free CheckUp and get a report on your risk for a fall in the next year.
  • Make an appointment with your healthcare provider and take your LIFE Fall Prevention Plan with you to discuss next steps if you are at risk for a fall in the next year.
